Transitioning out of Treatment – Transition Planning
A very important aspect of treatment, that can be overlooked in the beginning is transitioning out of it. We are fully committed to the success of each individual student. Finishing the program is a happy time, but it can also be quite frightening for some because this will mean going right back to the people and places that can be triggers. We want them to leave our program healthier and better prepared to re-enter everyday life, as well as, being able to make good judgments for their success.
Triumph is dedicated to helping each youth succeed beyond their time in the group home. The program is specifically designed for a lifetime of success with extensive transitional, aftercare planning with the youth and parents.
By incorporating other educational groups in the program, Triumph prepares each student with the ability to continue to find resources of support beyond their time with us. Both on and off campus sessions are provided by professional, organizations and/or volunteers such as Narcotics Anonymous (NA), Alcoholics Anonymous (AA), AR, and Boy Scouts of America.
